Medicare-Subsidised Physiotherapy for Chronic Conditions

Access expert physiotherapy with Medicare support through your GP Chronic Condition Management Plan (GPCCMP)

Strive Healthcare is proud to offer gap free physiotherapy through GP chronic condition management plans (GPCCMP). Our enhanced physiotherapy care plans go beyond routine preventive and wellness care, providing extended coverage with greater access to care management services. We recognize that many health issues today require more than a traditional one- size-fits-all approach. Therefore, our plans are tailored to address the specific needs of the individual and focus on proactive health management rather than reactive treatment only when acute episodes occur.

What is a GP Chronic Condition Management Plan (GPCCMP)?

A GP Chronic Condition Management Plan (previously known as a CDM plan or EPC plan) is a Medicare-supported referral pathway that allows eligible patients with chronic health conditions to access subsidised allied health services, including physiotherapy.

If you have a chronic or complex health condition that has been present (or is expected to be present) for six months or longer, and your GP believes physiotherapy would help manage your condition, you may be eligible to receive Medicare rebates for physiotherapy treatment while continuing to receive the same high-quality, personalized care from our physiotherapists.

At Strive Healthcare, we regularly work with GPs across Perth to help patients manage a wide range of chronic musculoskeletal, neurological, and mobility conditions through Medicare-supported physiotherapy.

Why Physiotherapy Matters for Chronic Disease Management

Chronic health conditions—whether arthritis, ongoing back pain, neurological conditions, balance problems, or post-operative limitations—can significantly impact your independence, confidence, and quality of life.

Physiotherapy plays a crucial role in chronic condition management by:

  • Reducing pain and stiffness through evidence-based treatment and exercise
  • Improving mobility and function so you can perform daily activities more easily
  • Building strength and endurance to maintain independence long-term
  • Preventing complications and reducing risk of falls, injury, or deterioration
  • Enhancing quality of life through improved movement, confidence, and participation in activities you enjoy
  • Reducing healthcare costs by preventing hospital admission, emergency department visits, and further medical intervention
  • Supporting self-management with education and strategies you can use independently

Research consistently shows that physiotherapy delivered early and regularly leads to better outcomes for patients with chronic conditions.

How to Access Medicare-Subsidised Physiotherapy at Strive Healthcare

Step 1 – Consult Your GP

Book an appointment with your GP and discuss whether you have a chronic health condition that might benefit from physiotherapy. Your GP will:

  • Review your medical history and current condition
  • Assess whether your condition meets the eligibility criteria (chronic condition lasting 6+ months)
  • Determine whether physiotherapy is an appropriate part of your management
  • Prepare a GP Chronic Condition Management Plan (GPCCMP) referral

What to bring to your GP appointment:

  • Any relevant scans, test results, or specialist letters
  • Current medication list
  • Information about your symptoms and how they affect daily life

Step 2 – Obtain Your GPCCMP Referral

Your GP will provide you with a physiotherapy referral as part of your GPCCMP. This referral:

  • Confirms your eligibility for Medicare-subsidised physiotherapy
  • Specifies your condition and management goals
  • Allocates your Medicare-subsidised sessions
  • Contains important information for your physiotherapist

You can receive your referral as:

  • A paper copy to bring to your appointment
  • An electronic referral to our clinic
  • A digital copy on email

Keep your referral safe—you’ll need it at your first appointment.
