What is an Care Plan?
An EPC (also known as a Chronic Disease Management Plan) is a Medicare program that allows patients with long-term or chronic health conditions to access allied health services, including physiotherapy.
With an EPC referral from your GP, you are fully covered for up to 5 physiotherapy sessions per calendar year here at Strive Physiotherapy.

The Care Plan is available for patients with chronic or complex conditions that have lasted (or are expected to last) for 6 months or more. Common eligible conditions include:
