What is an Care Plan? 

         An EPC (also known as a Chronic Disease Management Plan) is a Medicare program that allows patients with long-term or chronic health conditions to access allied health services, including physiotherapy. 

         With an EPC referral from your GP, you are fully covered for up to 5 physiotherapy sessions per calendar year here at Strive Physiotherapy.

Physiotherapy works best when treatment is consistent. Through the EPC plan, you get:

Who is Eligible? 

         The Care Plan is available for patients with chronic or complex conditions that have lasted (or are expected to last) for 6 months or more. Common eligible conditions include: 

• Arthritis and joint pain 

• Chronic back or neck pain 

• Diabetes-related mobility concerns 

• Post-operative recovery 

• Cardiovascular conditions 

Your GP will confirm your eligibility and provide a referral so you can access your covered sessions.
